Provides clarity regarding the current state of healthcare in America. As a physician in private practice with over a decade of experience in the regulatory arena at the local, state and national level, I found this book enlightening. It confirms what my observation and experience have borne out, which is regulatory capture by powerful interests. As a scientist I also found the book to be well researched and scholarly. I am sure that someone with a political or economic bias in either direction will pick and choose something to confirm their biases. Should be mandatory reading for all physicians.
The writing is great, well researched and scholarly. BUT the formatting of the ebook is not good, with awkard page breaks, weird paragraph or line breaks and terrible formatting of the footnotes. The awkward formatting makes it difficult to read. Probably some glitch in the digitization or Kindle app that could be easily fixed.
A brilliant review of the history of medical practices and how they evolved from cash based practices to insurance based practices.

Ms. Chapin’s bookincludes two features that make it valuable in such a study: describing the development, framework and later modifications of the “insurance company modelâ€, and describing the industry and policy interactions motivating how health product and service financing evolved from the early twentieth century to the 2010 Patient Protection and Affordable Care Act. On the first point, the expression “insurance company model†as used here is unique to this book and to closely related commentaries. Thus it forms an original template for evaluating industry practices and related policy developments over more than a century in the U.S. It also forms, as provided among the many endnotes, a basis for contrasting, at least in a general way, the U.S. system with some European models. For the reader looking for a concept, though not necessarily for the detailed description of how industry practices and policy initiatives proceeded through the decades, the book offers a valuable insight. As for this description, readers interested in the growth of health insurance coverage, both in numbers and in scope, will find a lot to consider in the interplay between the American Medical Association, insurance companies both for and non-profit, and political interests going at least as far back as the Truman administration. Here the analyses in chapters focused separately on the power and decline of the AMA, on health insurance industry associations, on the roles of Blue Cross and Blue Shield, on the attempts to contain costs, and on the development of Medicare, and then on Affordable Care Act legislation. The notes pages, some ninety pages in the paperback version, provide sources in support of the many observations provided through the previous eight chapters plus the introduction. A final remark on the title: are we discussion “ensuringâ€, as in assuring or guaranteeing, America’s health, or rather, the evolution of health insurance in the U.S. I think it’s the latter, and not to include measures aimed at public safety or providing clean air and water, in addition to the institutional arrangements for financing health products and services. This is not necessarily a major point, but an unavoidable question when re-reading the book’s title.

Not only is Ensuring America’s Health a contribution to historical and policy scholarship but, is highly recommended for readers who want to gain a deeper understanding of the complexities of the American health care system.In Ensuring America’s Health, Historian Christy Ford Chapin chronicles the historical development of insurance companies controlling the delivery and financing of health care. Through a well-written and persuasive argument, Chapin shows that interactions between the public and private sectors led to insurance companies becoming responsible for the delivery and financing of health care. In addition, that the “insurance-company model†explains the costly and fragmented American health care system.The characters in Chapin’s historical narrative are the American Medical Association, commercial insurers, non-profit plans (Blue Cross and Blue Shield), and federal policymakers. Chapin provides a detailed analysis of the structure, ideology, and political strategy of the AMA, commercial insurers, and non-profit plans. Most significantly, Chapin sheds a new light on the AMA. Since it is well-known that that AMA consistently opposed health care reform due to fears of “socialized medicine,†it has been assumed that the AMA was pro-free market. However, Chapin shows that this is not the case because they opposed and defeated other models of providing health care because they feared losing their professional autonomy to third-party financiers. The AMA eventually embraced health insurance because they feared federal government intervention, but the AMA dictated the particular model of health insurance.The evolution of the dominance of the “insurance-company model†was built up through a combination of indirect and direct actions by the federal government and the inefficiency of the model itself. In reaction to federal policymakers’ calls for health care reform, insurers and physicians response was to show that there was no need for federal government intervention but, the private sector could provide medical coverage for Americans. With the passage of Medicare, Chapin argues that this legitimized the “insurance-company model†for the program would be administered by insurers. While many scholars have claimed that health costs became a problem after the passage of Medicare, Chapin shows that this is incorrect. The “insurance-company-model†was instantly recognized as costly by federal policymakers. In addition, by the 1950’s, these inefficiencies propelled insurers to implement various cost-containment measures, which augmented their status to overseeing the delivery and financing of health care.
